I've not been able to find the cost of knighting for each age.

Is it simply the cost of a feast + the cost of the equipment?

That is how we do it, yes.

Book of the Manor has prices for it, too: Up to Conquest £30, Romance £35, Tournament £45, Grail & Twilight £55. If you go by the book and it is the eldest son, the price doesn't matter that much as it is paid by the peasants of the manor.
